First Visit

Your First Visit to Lakeside Audiology

We look forward to serving you. We value our relationship with our patients in a complete patient-centered practice. In order to better prepare you for your visit, please remember to bring the following to your appointment:

  • Your referral, if needed, by your doctor
  • Insurance card
  • Co-payment, co-insurance and/or deductible—payment is due at the time of service or per the terms of your insurance plan unless prior arrangements have been made.
  • Completed health forms
  • Health records or test results if possible
  • A friend or family member—we encourage you to bring a friend or family member if it would help facilitate the exchange of medical information and treatment recommendations

Cancellation Policy

If you need to cancel or reschedule an appointment kindly notify us at 585-577-4354  at least 24 hours in advance so that your time can be offered to another patient.
